Taking the Survey
Here are some options that may help you take the survey:

Low Vision Mode: Increases font size and color contrast. Activate this feature by clicking "Low Vision Mode" in the bottom right corner of the survey screen.

Spanish Version: You can switch between Spanish and English at any time during the survey by selecting the language in the bottom right corner of the screen.

Downloadable Survey: You can download the survey questions as a PDF or as a screen reader-accessible Word document. (The Spanish version is available upon request.)

Your Privacy
